Volunteers are the heart of our organization.
We enthusiastically welcome your involvement! There are many ways to volunteer, all requiring different commitments in time and talent. We strive to work with each volunteer individually in assessing the best way to help FPS pursue its mission.

Parental Consent: For Pete's Sake involves volunteers activities on and off the premises of the For Pete's Sake office. as the minor's parent/guardian, I hereby consent to his/her participation in the Activity. I understand that he/she will be provided with the proper training necessary to complete this Activity. I also acknowledge that this Activity may take place during school hours. I understand that he/she will not receive monetary compensation for the services contributed.

I am not aware of any physical or medical condition that would interfere with the child's ability to participate. If the child is injured or becomes ill and neither I nor any other parent/guardian identified below can be reached, I give For Pete's Sake permission to seek medical attention for the child.

I understand that the child may be photographed during the course of the Activity. I grant full and unlimited permission to For Pete's Sake Cancer Respite Foundation, and its agents and affiliates, to use the minor's name, photographs, or any other record of participation in this Activity in any broadcast, telecast, or other account of the Activity for publicity purposes, without compensation.

By signing below or submitting this form, I acknowledge that I am voluntarily participating in a For Pete’s Sake Cancer Respite Foundation (FPS) event, and understand that my participation may involve certain inherent risks, both known and unknown.

I hereby release, waive, and hold harmless For Pete’s Sake Cancer Respite Foundation (FPS), its employees, volunteers, directors, officers, and affiliates from any and all claims, injuries, damages, or liabilities that may arise from or be in any way connected to my participation in FPS events or volunteer activities.

I understand and accept full responsibility for any risks associated with my participation and agree that I am physically able and properly equipped to take part in the activity.

FPS agrees to indemnify, defend, and hold harmless all volunteers from any losses, damages, or claims arising from unforeseen incidents, provided the volunteer was acting in good faith and within the scope of their designated role.

In the event of an emergency, I authorize FPS to seek appropriate medical care on my behalf.

I also grant permission for my image, name, voice, and likeness to be photographed, videotaped, or recorded during any FPS event and used for promotional purposes, including but not limited to social media, print, and online content, unless I notify FPS in writing that I do not wish to be included.

I have read, understand, and agree to the terms of this volunteer agreement.
